We asked three AI engines the questions patients actually type, in three cities, and checked whether one med spa per city was named. In 5 of 45 answers it was. This was one run per question, so read it as a direction, not a forecast.

In 40 of 45 answers, the med spa we were checking was not named.

    What we tested

We ran 45 answers: 3 cities (Portland OR, Salem OR, Boise ID) x 5 patient questions x 3 engines (ChatGPT, Gemini, Perplexity).

The questions came from a sample of 15 Reddit thread titles from other cities. Topics: where to go for Botox, natural-looking filler, dermatologist vs med spa, lip filler injector, and Botox cost. Web search was on for ChatGPT and Gemini.

For each city we checked whether one specific med spa was named. The spas are not named here because they have not agreed to be named.

The same engine gave different results by city. ChatGPT named the Salem spa in 4 of 5 answers and the other two spas in none. Gemini named none of the three.

This table measures these five patient questions only. It is not a measure of any spa's overall visibility.

    What the engines look up

ChatGPT ran one rewritten search per question, for example "best Botox clinics in Salem, OR". Gemini ran three to four per question, for example "Botox Salem OR" and "dermatologist vs med spa botox".

Those search strings are what a page has to answer.

    Who gets cited instead

Sources Perplexity cited mostPerplexity shows its sources on every answer. ChatGPT and Gemini mostly do not, so treat this as a proxy, not their sources.

Directory and review sites recur. ChatGPT cited non-Google pages in 5 of its 15 answers. The rest pointed to Google Maps listings with Google ratings and review counts.

    One city, one scan

In a separate Local Falcon scan of Boise (keyword "med spa", 3x3 grid), ChatGPT named 18 different businesses across the 9 grid points, and no business appeared at more than 3 of the 9 spots.

That is one scan in one city. Do not generalize it.

    The takeaway

In this test, being named depended on the engine and the city. The pages the engines pulled from were directories and review sites. We have not shown that being listed on those sites changes who gets named.

    Methodology

    Sample

3 med spas, 3 cities, 5 questions, 3 engines (45 answers) · Portland OR, Salem OR, Boise ID · 2026-10-23

    How it was run

    1. Run through an n8n workflow using DataForSEO API models, web search on for ChatGPT and Gemini.
    2. A spa counts as named when its name appears in the first 3,500 characters of the answer text.

    Limitations

    • One run per question. AI answers vary from run to run.
    • Questions came from a Reddit sample from other cities.
    • Facts stated inside the answers, such as injectors, brands and prices, were not checked.
    • Perplexity sources are a proxy for the other engines.
    • The Boise Local Falcon scan is a single city.
